WHO WE ARE
A community network fighting HIV & TB in Kwale County.
CONPHAK is a County Network of People Living with HIV and those affected by HIV/AIDS in Kwale County, Kenya. We bring together communities, health workers and advocates to ensure everyone has access to equitable, dignified healthcare.
Backed by the Global Fund, AMREF Health Africa and the Kenya Red Cross Society, CONPHAK delivers seven targeted health programmes — from HIV care retention to community TB identification — reaching over 10,000 people across 100+ communities.

HIV Retention to Care
Supporting people living with HIV to remain in care through treatment literacy, peer support and defaulter tracing.

Awareness Drive
Reducing stigma and promoting open dialogue on HIV and health through community forums and structured dialogues.

PMTCT
Preventing HIV transmission from mothers to children through early testing, adherence support and continuous follow-up.

Capacity Building
Empowering communities through skills, leadership and service delivery via peer-led groups and health promotion.

Community System Strengthening
Community-Led Monitoring, Domestic Resource Mobilisation and Community-Led Advocacy for evidence-based engagement.

TB Prevention & Identification
Strengthening TB prevention through Community Health Promoters, contact tracing and treatment follow-up.

TB Case Identification & Care
Improving early TB detection through chest X-rays, targeted diagnostics and facility-based support groups.
