Who We Are
CONPHAK brings together people living with HIV, community health workers, and partner organisations to ensure no one in Kwale County is left behind. Our programmes address the full continuum of HIV and TB care, from prevention and testing to treatment adherence and community support.
What We Do
Seven evidence-based programmes addressing HIV, TB, and community wellbeing across Kwale County.

Supporting people living with HIV to remain engaged in care and adhere to treatment through active defaulter tracing and treatment literacy.

Reducing HIV/TB stigma and promoting health literacy through support groups, structured dialogues, and community forums across Kwale County.

Ensuring pregnant and breastfeeding women receive early testing, treatment adherence, and continuous follow-up. This service is delivered in Kilifi County.

Strengthening community health systems by empowering peer leaders and community health workers with skills and resources.
