HIV Retention to Care

Supporting people living with HIV to remain engaged in care and adhere to treatment through active defaulter tracing and treatment literacy programmes.

Community Awareness Drive

Reducing HIV/TB stigma and promoting health literacy through support groups, structured dialogues, and community forums across Kwale County.

PMTCT, Prevention of Mother-to-Child Transmission

Ensuring pregnant and breastfeeding women receive early testing, treatment adherence support, and continuous follow-up to prevent vertical HIV transmission. This PMTCT programme is delivered in Kilifi County.

Capacity Building & Service Delivery

Strengthening community health systems by empowering peer leaders, CHWs, and support groups with skills and resources for effective service delivery.

Community System Strengthening

Building resilient community systems through governance, advocacy, and coordination to address the social determinants of HIV and TB in Kwale County.

Community TB Prevention & Identification

Mobilising communities for TB screening, early identification, and linkage to care, ensuring no case goes undetected in hard-to-reach areas.

Increased TB Case Identification & Care

Scaling up TB case finding through community-led screening initiatives, contact tracing, and strengthened referral pathways to health facilities.
