Our Story

Built from the Community, for the Community

County Network of PLHIV in Kwale (CONPHAK) is a network community-based, public benefits organization established by people living with HIV in Kwale County who recognised that no top-down programme could replace the power of peer support and community mobilisation. It was founded to unite and empower individuals and groups living with or affected by HIV, TB, and malaria within Kwale County, the scope which has expanded to NCDs.

Over the years, we have grown into a trusted network partnering with local, national and international partners including Kenya Red Cross Society, AMREF Health Africa, the Global Fund, and the Kwale County Government to deliver life-saving programmes at the grassroots level.

Today, CONPHAK has become the most active community health network in the coastal region of Kenya, creating the space for the community’s engagement in regards to health services delivery: training peer leaders, tracing TB and ART treatment interrupters, supporting community-level HIV and TB prevention mechanisms, and, advocating for the rights of people living with HIV, TB, Malaria and NCDs.
